Request Your Medical Records

Our Release of Information (ROI) staff are ready to assist you with requesting your medical records. They also assist medical providers with completing forms for patients.

The ROI office can assist you with:

  • accessing your medical records
  • obtaining copies
  • requesting amends
  • completing forms for benefits, insurance and other reasons

Our ROI staff are experts in our patients' rights and their medical records. The ROI office is located on the second floor of the Bruce W. Carter VA Medical Center in room 2E100. To reach the ROI office, call 305-575-7000, ext. 3011. The fax number is 305-575-7236.

How to Request Information

Consent or authorization for release of information by an individual, physician or insurance company allows the hospital to send copies of information contained in a patient's medical record to another medical provider who may also be treating the patient, to other agencies/organizations for benefits consideration, or to the patient him/herself. Requests for records will take approximately 10-14 days to process. Because forms must contain an original signature, forms cannot be sent in via email.

Fees

There is no cost to send copies directly to another health care provider. If copies are for a patient's personal use, photocopying fees may be assessed.

The ROI office is open Monday-Friday, 8 a.m.4:30 p.m.
