Exit Interstate 95 at S.W. 10th Street. Drive west past the Military Trail intersection. After you take the second left turn, you'll see the VA Primary Care Clinic banner in red.
Services Available
The Deerfield Beach CBOC is open Monday through Friday, from 8:00 a.m. to 4:30 p.m. It does not offer emergency services. Those services provided are:
- Primary care
- Primary preventive care
- Electrocardiogram (EKG)
- Laboratory (phlebotomy)
- Ancillary testing
- Internal Medicine
- Radiology (x-ray)
- Women's Health Care
- Medication management and monitoring
- Vaccinations
- Alcohol and drug screening
- Pharmaceuticals
Each veteran will have his/her health care concerns handled by our primary care teams who will then become their personal medical provider. All patients are encouraged to call 954-570-5572 between 8:00 a.m. and 4:30 p.m. Monday through Friday, except holidays, if they have any health questions. These questions will be answered by the patient's primary care team.
Initial prescriptions for medications will be provided during the clinic visit. Medications for continuous therapy will be provided through the prescription mail out program. Requests for prescription refills will be mailed to Pharmacy Service, Miami VA Healthcare System, or through the automated telephone system. Medication refills or information about your VA prescribed medications can be obtained by calling the Miami VA Healthcare System Pharmacy at 305-575-3133 or 1-888-276-1785.
Registration and Enrollment Information
Pre-registration at this clinic is required. Patients not previously registered may do so at this location and will be given an appointment for a later date. |
